Hire someone to send the messages, or pay the platforms to send the people? The difference is the shape of the cost, and it only shows when you divide by calls that happened.
What does an appointment setter actually cost a coach?
A setter costs a fixed amount every month whether or not calls book: $16.08 an hour base in PayScale's May 2026 survey of 76 profiles, or $45,380 a year ($22.97 an hour) across the 10,000 job-board salaries Talent.com aggregates for 2026, plus commission PayScale's profiles put at $9k to $15k a year.
That is $3,782 a month at Talent.com's average, or about $1,394 for 20 hours a week at PayScale's base. Commission-only setters are paid per call that shows plus a share of what closes; no survey pins that fee. The salary also hides capacity: a cold list buys the same hours and fewer calls, and when the setter leaves, the pipeline leaves too.
What do paid ads cost per booked call for coaches?
Ads cost per lead, not per month: LocaliQ's 2026 benchmarks put the all-industry cost per lead at $27.39 on Meta lead campaigns and $66.69 on Google search, and a booked call costs a multiple of that, because not every lead books.
Neither report has a coaching row; the nearest search rows are Business Services at $93.69 and Education and Instruction at $77.48. A lead here is a form fill, and your number will not be the average: the week of 13 July 2026 our Meta scaling campaign for The Kitchen Store in Los Angeles ran at $47.51 per lead against $64.63 across the account. The shape is the point: twice the calls means more budget, not another person.

Which books calls that actually show up?
Calls prospects book for themselves show up far more often than calls booked for them: Focus Digital's September 2026 compilation puts no-shows at 8% for instant scheduling, 33% for paid lead forms and 42% for outsourced appointment-setting agencies.
Timing matters as much as channel: GrowthSpree's May 2026 benchmarks show 82% to 92% of same-day bookings showing up against 35% to 48% of bookings made more than two weeks out.
The fix is the same on both routes: a near slot, chosen by the prospect, and a call inside five minutes. The InsideSales.com and MIT study of more than 15,000 web leads found the odds of qualifying a lead drop 21 times when the first call comes at 30 minutes instead of 5. That is the 5-minute follow-up rule.
What is the real cost per booked call? A worked example
On the assumptions below, a call that happens costs $214 through a setter and $167 through ads, or $121 once the ads feed an instant booking page.

The setter route
$3,782 a month at Talent.com's average. Assume 40 booked calls and a $50 bonus per call that shows. At Focus Digital's 42% no-show rate, 23 happen. Cost $4,932: $123 per booked call, $214 per call that happened.
The ads route
$4,000 into Meta lead campaigns at $27.39 buys 146 leads. Assume one in four books: 36 calls. At 33% no-shows, 24 happen: $167 each. With instant same-day booking and a five-minute callback, the 8% rate gives 33: $121. On Google search at $66.69 the same budget buys 60 leads, so the cost runs higher.
Now double it
The setter is at capacity, so the second 40 calls need a second setter at the same $214. Ads at $8,000 buy the second 146 leads at close to the same price, until the creative tires.
